Submit Your Wedding Schedule

Please complete this online form at least 3-4 weeks before your wedding date, so we can properly plan out your musical selections and flow of your event. If there is anything on this form you are unsure of, please email info@mixjockey.ca so we can assist you with any questions you may have. We can not guarantee music, special requests or requested performers if your wedding schedule is not returned by the deadline.

We will review the wedding schedule together with you (via phone call) 1 week before your event, to ensure we understand all your requirements and clarify any points.

You can save your entries and come back to it later. Once you start, you have 30 days to complete the form.  If choose to save and continue later, you will receive an email with a link to access the wedding schedule you started filling out.
